Registered Nurse - Home Care
Qualifications
Required
- Bachelor's degree or higher

- Valid Registered Nurse (RN) in the state of New York (NY)
Overview
What you need to know:
- Extensive 2-4 weeks of orientation
- Monday - Friday schedule 8:30am-4:30pm (must also work one weekend per month)
- 5-7 home visits per day - Must have RN license in NY
- Must have a car, a NY issued driver's license, and proper auto insurance
- Parking and other expenses necessary will be reimbursed
- $12,000 sign on bonus. $5000 will be paid out at the end of the first month and the remaining $7000 will be paid out after 12 months.
- Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost-effective multidisciplinary health care services for a caseload of patients/clients consistent with philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision.
- For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants.
- Also responsible for adhering to NFP national guidelines. As part of our client's CHHA Team, Staff RNs render short-term rehab care to end-of-life patients in the home setting.
With the support of the field team--including RNs, LPNs, supervisors, and other providers in the collaborative care team--RNs meet patients in their most comfortable setting to provide quality, comfort-based care and education.
Responsibilities:
- Render care in accordance with our client's policy, goals, objectives, philosophy, and Standards of Nursing Practice.
- Direct, hands-on patient care, including wound care; management of IV's and catheters; administration of medications
- Case management responsibilities, including scheduling; assessments; assessments; referrals; documentation.
- Assess physical, psychological, and social needs of patients and environmental conditions in order to develop an effective plan of care - Travel to patients' homes to render care
- All types of patient visits, including start of care and revisits
- Report to regional leadership
- Participation in regional team activities, including "buddying" with selected staff during orientation and providing feedback on performance
- Documents content and process of all services provided in accordance with government regulations/requirements, and our client's Health policies, practices, and procedures - Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care
Integrated Pharmacist
Must-Haves
1. Clinical Pharmacy Residency, PGY1 minimum
2. Clinical Pharmacy Specialty experience in General Pediatrics, NICU or PICU
3. Acute care inpatient pediatric experience is required.

Minimum Qualifications
Education: Graduate of an accredited School of Pharmacy
Licenses/Certifications: Licensed as a Registered Pharmacist in the State of Texas; Pharmacy Sterile Product certification through an in-house program or other accredited program is required if incumbent compounds sterile products.
Experience / Knowledge / Skills:
Two (2) years of experience as a hospital Pharmacist (or at discretion of System Executive, System Pharmacy) with knowledge of the unit dose medication and I.V. admixture processes

Principal Accountabilities
Fills and labels prescriptions and provides drug-related information and education for in-patients, out-patients, hospital employees and medical staff.
Dispenses controlled substances and maintains appropriate inventory records.
Supervises the activities of the pharmacy technical staff and Mentors or trains pharmacists, pharmacy students or residents.
Participates in the Medication Use Evaluation Program and the Process Improvement Program.
Maintains medication profiles, reviews profiles for drug related problems, and provides consultation on drug research.
Maintains the scheduling and preparation of I.V. solutions and additives.
Initiates reports of medication errors or adverse drug interactions.
Assumes full responsibility for department management in the absence of the Pharmacy Manager or Lead Pharmacist.
Works with other professionals to generate newsletters. Drafts policies or procedures for review by Pharmacy Director.
Performs clinical interventions, which may include medication profile review, IV to oral conversion, renal dosing adjustment, antimicrobial streamlining, patient counseling, obtaining medication histories, vancomycin/aminoglycoside dosing, therapeutic drug monitoring, and participation in multidisciplinary rounds.
Documents clinical interventions.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
Other duties as assigned.
Clinical Pharmacist Specialist-PICU
Our Pharmacy team seeks a residency trained Clinical Pharmacist to join our 32-bed Pediatrics Intensive Care Unit. This position will serve as a clinical specialist for our pediatrics critical care team.
Must-Haves
1. Doctorate in Pharmacy is required (PharmD)
2. Must have PGY2 Residency training OR Fellowship to be considered for this role. We are unable to make exceptions.
3. Related experience: Clinical pharmacy experience in pediatrics, PICU is highly desired. Will consider residency rotation.
4. Must be a clinical specialist in an inpatient hospital pharmacy environment.
This position requires PGY2 training or Fellowship.
Minimum Qualifications
Education: Doctor of Pharmacy degree; Completion of ASHP accredited pharmacy practice residency or fellowship or demonstration of equivalent experience preferred
Licenses/Certifications: Licensure in State of Texas; Pharmacy Sterile Product certification through an in-house program or other accredited program if appropriate
Position develops, implements, and monitors clinical pharmacy programs to improve patient care with an additional focus on decreasing cost; provides clinical services to patients and professional staff; interacts with medical staff on drug utilization and cost containment. May be asked to assist in supervising the activities of all department employees in consultation with the department Director and/or Pharmacy Manager.

As our client's Hospice Clinical Supervisor, you will play a crucial role in ensuring the delivery of outstanding care to our patients. You will lead and support our clinical staff, ensuring compliance with regulations and guidelines while fostering a compassionate and supportive environment.
Key Responsibilities
Ensure productivity goals of clinical staff are met.
Ensure case management guidelines are being followed by each team member.
Assign patients to multidisciplinary team and adjust schedules as needed.
Serve as the first line for resolution of concerns and issues for the staff.
Train, develop, and mentor staff, including creating and recruitment, administering orientation process and patient assignment, providing employees with continuous training and education, and performing employee reviews, exits, counseling, and meetings.
